Weekly Success Checkpoint

Instructions: Please fill out the progress form each week at least 24 hours before our group session. You’ll get a copy sent to you and one sent to Stella. Save each form in a folder so that you can see the previous week’s content. The easiest way to fill out the quarterly goals question is to copy and paste from your previous progress form. This will keep you on track, focused and updated. As you progress through your months, your quarterly goals may change. That’s fine.
